LONG-TERM CARE

G.E.M.'s Long Term Home Health Care Program provides professional in home supervision of the Home Health Aide to the chronically ill or disabled and to the elderly who might otherwise require nursing home placement.  G.E.M. provides:

• Registered nurses who provide health assessments and education about disease management and medication regiments
• Personal care aides
• Home health aides
• 24 Hour on call service
• Respite Care

Referrals

• Physical, occupational and speech therapy
• Medical supplies, equipment assessment and safety evaluations
• Registered dietitians and nutritionists
