PATIENT'S RIGHTS POLICY
Cients have the right to receive the
Client Bill of Rights and Responsibilities.
Policy

Our goal is to promote the
client's interest and well being.

To ensure that client's civil and
religious liberties are not infringed.
Procedures
-
Each client admitted to G.E.M.
Health Care Agency is given information regarding
their bill of rights and responsibilities, advance
directive, aide assignements, agency material and if
applicable client's pay agreement.

The Home Health Care Director or
Supervising Nurse will:

Review the above information
with the clients.

Request a signature of the
client, family, or significant other.
What to do if you have a
complaint?
You are able to file a complaint
either to G.E.M. Health Care Agency by calling us at

The Agency provides
the health and home care needs with
Registered Professional Nurses (for
Assessment), Personal
Care Aides, Home Health Aides, and Homemakers. The Care Plan
will be individualized according to your
medical needs, availability of service,
and the physician’s recommendation.
